So, do they really need this info? Personally, if this all makes sense....why would I stop at just residence, age and vax-status? I'd ask if you had a dog or cat....if you were a smoker....if you sipped more than two beers a day.....if you sleep-walked....if you were into weird fantasy sex stuff....if consumed more than 1,800 calories a day....if you were far-right-wing....if you were into South Korean zombie movies....if you were did smoke some occasional cannabis....and if you knew anyone who was paranoid schizophrenic.
If you don't answer the vax-status, age or residence question? Well....no one has said for sure what the punishment will be. Maybe the nurses refuse to admit you into ICU and you just go to regular quarantine.
If you gave a fake answer....saying you were from Paris, 48 years old, and vax'ed with six different booster shots? As long as you don't sign the document....legally, you'd be OK. If you had to sign the document? Just sign 'Winston Smith' and see how the hospital staff reacts (from the book '1984' chief character).
Most of these people arriving by ambulance with serious bouts of Covid....NOT in condition to really answer stupid questions? Well....yeah, that probably represents a major problem. 'Brain fog' is one of the elements of severe Covid, so you could make up an entire story....fraudulent in nature.....over your details, and not remember much of anything.
That's really one of the major problems in German society....for the past two-thousand years. Once you start suggesting regulating things....it always get more messy....year by year.
